About Form 6
Form 6 — Annual Return by a Non-Profit Organisation must be completed and submitted in person at the Registrar General’s Office each year. It has 8 questions. Questions 1–4 cover your organisation’s basic identification details.
Question 1 — Name of NPO
Enter the full legal name of your NPO exactly as it appears on your Certificate of Registration (or other registration documents for non-incorporated organisations). Do not abbreviate.
Question 2 — NPO Registration Number
Enter the Non-Profit Organisation Number from your NPO Registration Certificate. This is the number assigned when you registered under the NPO Act, not your Companies Act incorporation number.
Question 3 — Address
Enter the full address of the registered office of the NPO.
Question 4 — Anniversary Date of Registration
Enter the anniversary date of your NPO registration for the year in which the return is being filed. For example, if your organisation was registered on 15 March 2022 and you are filing for 2025, enter 15 March 2025.
