What is a Controller?
A controller is any person who exercises control over the NPO, whether directly, indirectly, or through other means. All controllers must be declared on Form 1 and in every Annual Return.
Required Information for Each Controller
For each controller, you must state:
- Full name — first given name, middle name, and surname
- Full street address — including building number and unit number (if a multi-dwelling unit)
- Occupation — stated clearly, with area of specialty where possible (e.g. electrical engineer, not just engineer)
- Nationality
- Telephone number(s)
- Email address
- Basis of control — the basis upon which the person is considered a controller (e.g. director, majority shareholder, trustee)
Multiple Controllers
If your NPO has more than one controller, complete all the above details for each person separately.
