These Sections Reflect Current Status
Questions 5–8 should reflect who holds each role at the date you are filing the return, not who was listed in your original registration. If your controllers, members, founders, or beneficiaries have changed, record the current details here.
Question 5 — Controllers
For each controller at the date of the return, state:
- Full name (first given name, middle name, surname)
- Full street address including unit number
- Occupation with area of specialty (e.g. electrical engineer)
- Nationality
- Telephone number(s)
- Email address
- Basis upon which the person is considered a controller
Question 6 — Members
For each member (natural person or corporation), state full name, address or registered office, occupation or corporate status, and nationality or jurisdiction of incorporation.
Question 7 — Founders
For each founder (natural person or corporation), state the same details as for members: full name, address or registered office, occupation or corporate status, and nationality or jurisdiction of incorporation.
Question 8 — Beneficiaries
State the full name, address, occupation, and nationality of each beneficiary, or describe the class of beneficiary (e.g. “children in underserved communities in Trinidad and Tobago”).
Signature
After completing all 8 questions, the controller must sign the form indicating the capacity in which they are signing.
