When to Use This
Use Form 4 Items 4 and 5 when a controller’s details have changed, or when a new controller is being added to your registered NPO.
Item 4 — Changes to Existing Controller Details
Only complete Item 4 for the specific details that have changed. For each change, state:
- The controller’s full name
- The detail that has changed (name, address, occupation, nationality, or nature of control)
- The new information
- The effective date of the change
For a name change, also state the authority by which the name was changed (e.g. deed poll, marriage certificate).
Item 5 — Adding a New Controller
To add a new controller, complete all of the following in Item 5:
- Full name (first given name, middle name, surname)
- Full street address including unit number
- Occupation with area of specialty (e.g. civil engineer)
- Nationality
- Telephone number
- Email address
- Basis on which the person is to be considered a controller
Submit Form 4
Bring the completed Form 4, signed by the controller, to the Registrar General’s Office in person.
